What is 245D?
245D refers to the Minnesota licensing standard for providers delivering Home and Community-Based Services (HCBS) under state waivers such as:
- CADI – Community Access for Disability Inclusion
- BI – Brain Injury Waiver
- DD – Developmental Disabilities Waiver
- EW – Elderly Waiver
We work with case managers, families, and clients to develop person-centered support plans focused on independence, safety, and inclusion.
